| **CHRIST THE KING CYO BASKETBALL SIGNUPS**
Sign ups for the 2008-09 basketball season will be held on Sept 23, 25 & 29 at Christ The King Middle Building Gym from 6:30-7:30 pm. In time saving measures, please click on the lines below. You will need the registration & medical matters forms (there are 2 forms attached to this file entitled SIGN UP FORMS) and the FAMILY PLEDGE form filled out completely, which includes all health insurance information. Basketball is open to all children in our three parishes (Holy Trinity, St John's and St. Peter's) from grades 1-12. The fees for this years program will be $40 each for the 1 & 2 grade program, $65 each for grades 3-12. There are multilple family discounts (Those from within the same family) so please contact Gloria Foster at 610-502-1276 for those discounted figures. Having the forms filled out in advance will save time in the sign-up process. This year the CYO will NOT be conducting any fundraising efforts.
